Group Health Individual Medical Plans Rates
Establishing your monthly rate
The following rates apply to each person enrolled in a plan and vary depending on the effective date of coverage, tobacco use, age range, zip code and the chosen deductible amount. To determine the monthly rate for each plan:
- Find the name of the plan you wish to enroll in. (Only one plan per application may be selected)
- Choose one of the deductible amounts available for that plan.
- Identify the individual rate for the benefit plan based on the chosen deductible amount, your current age range, zip code and use of tobacco products.
- Repeat step 4 for each person who will be covered under this plan (if applicable).
- Add up all applicable rates. This is your total monthly rate for the plan you selected.

Important notes:
- Our benefit plans for individuals and families are available to permanent
Washington State Residents, except those eligible for Medicare.
- Eligible family members include you, your spouse, and unmarried dependent children under age 23 who are primarily dependent on you for support.
- The child rate listed is "per child".
- The deductible amounts listed are "per person, per calendar year".
- If anyone covered under your plan (including yourself) has used tobacco products within the past 12 months, the smoker rate will apply only for that person.
- When you fill out your application for coverage, you can elect to pay your rate through an automatic bank withdrawal or receive a monthly billing.
